College Profile Materials

We ask that you complete the following intake forms and share some materials with us. Our counselors will review this information prior to your session.

Part Two: Academic Profile Materials

  1. Transcripts from 9th grade to present
  2. Working Resume- a list of activities, honors, distinctions, sports, fine and performing arts, clubs, work, internship experience, volunteering, hobbies, and passions
  3. Two recent writing sample from different classes (for example, a history and an English paper)
  4. Any and all testing (MCAS, PSAT, PACT, SAT/ACT, SAT, Subject Tests, etc)
  5. Please let us know if your student has an IEP report, 504 plan and/or neuropsychological testing evaluation
